South Lanarkshire Reachout Service provides Assertive Outreach and Intensive Case Management to support people with problematic alcohol and another drug use.

We understand the assertive outreach approach to be at the heart of a person-centred and flexible model of service. This model is designed to take services throughout South Lanarkshire to reach people who have traditionally struggled to engage, building a trusting relationship during this period.

The service will operate throughout the whole of South Lanarkshire with offices in Cambuslang – Hamilton – East Kilbride and Clydesdale.

The service model will focus on three key groups:
• Individuals who do not engage at any level;
• Individuals who sporadically engage therefore proving difficult to achieve desired outcomes;
• Individuals who partially engage where the focus is purely on medical treatment;
